*Our office is closed daily between noon and 1pm for lunch and our office observes Federal Holidays.

Appointments
We offer prompt scheduling, evening hours for your convenience, and same day appointments are often available.

• Please bring a Photo ID as well as a copy of your Current Insurance Card.
• Copay must be paid the date of service.
• Minors will not be seen without a parent or legal guardian present
• New Patients must arrive at least 20 minutes early to fill out New Patient forms or Fill out New Patient Forms prior to your visit.
• If you are late for your appointment, you may be asked to reschedule.
• Please check your insurance plan to ensure our physicians are on your plan
• There is a $25 No Show Fee for missed appointments. Please contact our office prior to your appointment if you will need to cancel or reschedule your appointment.

Insurance
Our office accepts Medicare and a variety of PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier. It is your responsibility to verify your coverage benefits with your insurance carrier.

Our Office is equipped with Digital X-ray technology, Diabetic Shoe Program, Electronic Insurance Claim Submission, and an Electronic Medical Records System.
